FSU Passport Acceptance Facility
Students, faculty, staff, and other members of the local community can apply for a US passport book and/or passport card directly through FSU's Passport Acceptance Facility in the International Programs office. The FSU Passport Acceptance Facility is appointment only; walk-ins cannot be accommodated. You don't need to be affiliated with FSU to make and appointment.
As a Passport Acceptance Facility, Florida State University International Programs is authorized by the U.S. Department of State to accept applications for U.S. passports. Our office verifies your identity, reviews your application for completeness, and submits your application and supporting documentation to the U.S. Department of State for processing.
Please note that our office does not issue passports or determine an applicant's eligibility. Only the U.S. Department of State has the authority to approve or deny a passport application and issue a U.S. passport.

Services
Florida State University Passport Services provides the following services by appointment only:
- Acceptance of U.S. passport applications for first-time applicants and individuals who are not eligible to renew their passport.*
- Biometric passport photo services that meet ICAO requirements.
*If you are unsure whether you are eligible to renew your passport online or by mail, please consult our renewal eligibility information before scheduling an appointment with our office. Most undergraduate students are ineligible for passport renewal.
Who Can Apply
Our passport acceptance services are available to all U.S. citizens applying for a first-time passport or who are otherwise ineligible to renew their passport by mail. You do not need to be affiliated with Florida State University to apply for a U.S. passport through our office.
In addition to passport acceptance services, we offer passport and visa photo services to the public. Our professional biometric photo system can produce photos that meet the requirements for U.S. passports as well as the passport and visa photo specifications for most foreign countries.

Inside the International Programs Office
The FSU Passport Acceptance Facility is located in University Center A, in the University Center complex which surrounds FSU's Doak Campbell Stadium. Directional signage is posted throughout the complex to help visitors locate each University Center building.
University Center A is the easternmost building in the complex, closest to the interior of FSU's campus.
Once inside University Center A, take the stairs or elevator to the 5th floor to the FSU International Programs office.
You can enter the IP office through one of two entrances: through the College of Motion Picture Arts side or through the main administrative entrance in University Center A which also houses FSU Admissions, Registrar, and Office of Student Finance.
Parking
FSU parking can be limited when classes are in session, so we strongly recommend arriving early to ensure you have sufficient time to locate parking and arrive at your appointment on time. Be sure to allow extra time for parking during the fall and spring semesters.
FSU Students, Faculty, & Staff
FSU students, faculty, and staff should use the regular parking lots available per your university parking permit.
Non-FSU Visitors
Parking is available in the stadium's visitor lot. Bring your parking ticket to your appointment so we can validate it for you before you leave. For directions visit the FSU Parking Website. If you have specific questions about accessible parking accommodations or parking options, be sure to contact FSU Transportation & Parking Services prior to your appointment.
Additional paid parking (green parking spaces) are available around the University Center complex for visitors without an FSU parking permit. Payment for these spaces must be made through the designated mobile parking app. Note: we cannot validate or reimburse parking fees paid through the app.
Note: Visitors who park in ADA parking spaces on campus are not cited if you have a valid state-issued ADA disabled placard or plate.

The FSU Passport Acceptance Facility is open Monday through Friday by appointment only.
Appointments open no more than three weeks in advance. If there a no available appointments at this time, check back the next business day.

- If you do not arrive within 10 minutes after the start of your appointment, your appointment will be canceled.

Processing Times & Urgent Travel Options
Processing Times
Routine Processing: 4-6 weeks
Expedited Processing: 2-3 weeks (Additional fee required)
Shipping: It may take up to 2 weeks for applications to arrive by mail at a passport agency or center from the FSU Passport Office, and up to 2 weeks for you to receive a completed passport in the mail after it is printed. Expedited shipping options are available for a fee (see "Fees" section of this page for details).
Note that passport processing times are subject to change. Refer to the U.S. Department of State website for the most up-to-date processing times.
Urgent travel
If your departure date is less than two to three weeks away, our office may not be the best option for obtaining your passport in time for travel. As a Passport Acceptance Facility, the fastest service we can offer is expedited processing.
If you need your passport sooner, you may wish to consider one of the following options:
Regional Passport Agencies
The U.S. Department of State operates Regional Passport Agencies that can provide expedited, and in some cases same-day, passport services for eligible travelers. Appointments are generally available only for individuals who can demonstrate:

Urgent Travel: Proof of international travel within the next 14 calendar days (or within 28 days if a foreign visa is also required).

Life-or-Death Emergency: International travel within the next 14 days due to a qualifying life-or-death emergency involving an immediate family member.
The Regional Passport Agencies closest to Tallahassee are located in Atlanta, Miami, and New Orleans. Additional information about eligibility, required documentation, and scheduling an appointment can be found through the U.S. Department of State's Urgent Travel and Life-or-Death Emergency resources.
Registered Passport Courier Services
Applicants who do not wish to travel to a Regional Passport Agency may also wish to consider using a registered passport courier service. These companies submit passport applications on behalf of applicants directly to regional agencies and may be able to obtain passports more quickly than expedited service.
Neither Florida State University Passport Services nor the U.S. Department of State endorses or recommends a specific courier service. Applicants are responsible for researching any courier they choose to use, verifying its legitimacy, and paying any associated service fees. Additional information about registered passport courier services is available through the U.S. Department of State.
Passport Application Requirements
Be sure to review the following information carefully to ensure you are fully prepared for your appointment. It is your responsibility to review these materials, gather all required documentation, and contact Florida State University Passport Services with any questions before arriving.
Applicants who do not arrive with all required forms, documentation, and fees will be unable to submit their passport application and may need to schedule a new appointment.
All Applicants
All applicants applying for a U.S. passport through Florida State University Passport Services must provide the following forms, documentation, and fees at the time of their appointment. Please review each requirement carefully to ensure you arrive fully prepared. If you do not bring all required materials will be unable to submit your passport application.
Requirement 1: DS-11 Application Form
All applicants must complete Form DS-11: Application for a U.S. Passport using the U.S. Department of State's online Form Filler. This is the official application used to request a U.S. passport.
Please note the following when completing your application:
- Print the completed application single-sided on standard 8.5" × 11" paper.
- Do not sign the application. You must sign Form DS-11 in the presence of a Passport Acceptance Agent during your appointment.
- If you do not have access to a printer: you may complete a blank DS-11 application by hand during your appointment. If you plan to handwrite your application, please arrive approximately 10 minutes early to allow sufficient time to complete the form before your appointment begins.
- You will be asked whether you would like to apply for a passport book, passport card, or both. Compare the book and card using the U.S. Department of State resource.
- You will also be asked to select either a standard (28-page) or large (52-page) passport book. There is no additional cost for the large book, so applicants who travel internationally frequently may wish to select this option.
The remainder of the application requests your personal information, including your name, date of birth, contact information, mailing address, Social Security number, information about your parents (including their names, dates of birth, places of birth, and your mother's maiden name), marital status and spouse information (if applicable), appearance, occupation, anticipated international travel, emergency contact details, and information about your most recent U.S. passport.
If your previous passport was lost or stolen, provide as much information as possible. If you need additional space to explain the circumstances or provide requested information, you may attach a separate sheet of paper to your DS-11 application.
Important: If the Department of State's Form Filler recommends a different application — such as Form DS-82 (Passport Renewal) or Form DS-5504 (Passport Corrections, Name Change, or Limited Passport Replacement) — the information you entered indicates that you may not need to apply in person. In most cases, these applications can be mailed directly to the U.S. Department of State by following the instructions provided with the appropriate form. Questions about these forms can be emailed to IP-Passports@fsu.edu.
Requirement 2: Evidence of U.S. Citizenship
All applicants must provide original evidence of U.S. citizenship. This documentation is required to verify your eligibility for a U.S. passport and must be submitted with your application. Photocopies are not acceptable as proof of citizenship. Laminated documents are not accepted.
You must provide one of the following:
- An expired or current U.S. passport book or passport card.
- An original or certified copy of your U.S. birth certificate.
- The birth certificate must be a long-form birth certificate containing, at a minimum, the applicant's full name, date and place of birth, parent(s)' names, the registrar's signature, and the official seal of the issuing authority.
- Short-form birth certificates or wallet-sized birth certificate cards are not acceptable.
- A Certificate of Naturalization
- A Certificate of Citizenship
- A Consular Report of Birth Abroad (CRBA) or Certification of Birth Abroad
Please note that your original proof of citizenship will be submitted with your passport application and returned separately by the U.S. Department of State after processing.
Requirement 3: Photo Identification
All applicants must provide proof of identity at the time of their appointment. The U.S. Department of State maintains a list of primary and secondary forms of identification.
Applicants should bring at least one primary form of identification. If you do not have a primary ID, you must present two secondary forms of identification.
The preferred primary form of identification is a fully valid, in-state driver's license.
Please note the following:
- In-state learner's permits are considered secondary identification, not primary identification.
- In-state non-driver identification cards are considered secondary identification, not primary identification.
- Out-of-state driver's licenses are also considered secondary identification.
If you present one of the forms of identification listed above, you must also provide an additional secondary form of identification.
For Florida State University students, faculty, and staff, an FSUCard may be used as a secondary form of identification. Members of the public who are not affiliated with Florida State University should review the U.S. Department of State's list of acceptable secondary forms of identification linked above to determine which additional document(s) they may use.
Requirement 4: Passport Photo
All applicants must submit one (1) 2 × 2-inch passport photo that meets the quality standards established by the U.S. Department of State and the ICAO.
Florida State University Passport Services offers passport photo services during your appointment for $10.74. If you choose to have your photo taken by our office, no additional preparation is required.
Applicants who prefer not to use our photo services are welcome to obtain their passport photo from another provider. However, please note that unacceptable passport photos are one of the most common reasons passport applications require correction, which can delay the processing of your application until an acceptable replacement photo is provided to the U.S. Department of State.
For the best experience and to minimize the risk of delays, we recommend having your passport photo taken by our office.
Requirement 5: Fees
All applicants are responsible for bringing the required passport fees to their appointment. In our experience, arriving without the required fees or with payment in an incorrect format is the most common reason applicants are unable to submit their passport application during their scheduled appointment.
Passport applications require two separate payments:

U.S. Department of State (Federal Government): This payment covers your passport application fee and must be paid by check or money order made payable to the U.S. Department of State. Cash and credit/debit cards CANNOT be accepted for this payment.

Florida State University: This payment covers the $35.00 passport execution (acceptance) fee and, if applicable, the $10.74 passport photo fee. This payment MUST be made by credit or debit card.

Applicants Age 16 or 17
Additional Requirement 1: Parental Awareness
Applicants aged 16 and 17 qualify for an adult (10-year) passport. However, since they are still under 18, they are required to demonstrate parental awareness at the time of their appointment. Parental awareness can be demonstrated by any one of the following:
- A legal parent or legal guardian appearing with you in person when you apply for your passport. The agent or employee accepting the application will ask your legal parent or legal guardian to sign Form DS-11. Bring a photocopy of that parent or guardian's ID.
- Submit a signed note from your legal parent or legal guardian with a photocopy of their ID.
- Submit proof that your legal parent or legal guardian is paying your application fees (Example: parent or guardian name is written on the check or money order).
- We may ask you to submit a notarized statement from your legal parent or legal guardian (e.g., on Form DS-3053) which states he or she supports you receiving a passport. The statement must be accompanied by a photocopy of that parent or guardian's ID.
Applicants Under Age 16
Applicants under the age of 16 qualify for a child passport, which is valid for five years. In addition to the standard application requirements, applicants under 16 must satisfy the following parental requirements:
Additional Requirement 1: Parental Consent
Both legal parents or legal guardians must provide consent for the issuance of the child's passport.
- Both parents or legal guardians should appear in person with the child at the passport appointment and sign Form DS-11 in the presence of the Passport Acceptance Agent.
- If one parent or legal guardian cannot appear, the absent parent or guardian must generally submit Form DS-3053 (Statement of Consent) or other documentation as required by the U.S. Department of State.
- Certain circumstances, such as sole legal custody or other special family situations, may require additional documentation. Email IP-Passports@fsu.edu if you have questions or need any assistance.
Additional Requirement 2: Evidence of Parental Relationship
Applicants must provide documentation establishing the relationship between the child and the parent(s) or legal guardian(s) providing consent.
Acceptable examples include:
- An original or certified U.S. birth certificate
- A Consular Report of Birth Abroad (CRBA)
- An adoption decree
- A court order establishing legal guardianship
- Other documents accepted by the U.S. Department of State demonstrating the legal parent-child relationship
Additional Requirement 3: Proof of Parental Identity
Each parent or legal guardian providing consent must present valid identification.
- Parents or legal guardians appearing in person must present an acceptable form of identification. See Requirement 3: Photo Identification for details about acceptable identification.
- If submitting Form DS-3053, the absent parent or guardian must include a photocopy of the front and back of the identification used to notarize the form.
Because passport applications for children under 16 often involve additional documentation, parents and legal guardians are encouraged to review the U.S. Department of State's guidance before their appointment and contact Florida State University Passport Services by emailing IP-Passports@fsu.edu with any questions.
Unique Passport Needs
The U.S. Department of State provides additional guidance for applicants whose passport needs extend beyond the standard application process. If any of the situations below apply to you, please review the applicable Department of State resource before your appointment.
- Applicants with Disabilities: Information is available regarding accessibility, accommodations, and passport services for individuals with disabilities.
- Sex Markers: The Department of State provides guidance for applicants requesting a passport with the sex marker that best reflects their gender identity.
- Second U.S. Passport Books: In limited circumstances, eligible travelers may apply for a second valid U.S. passport book, such as when frequent international travel or conflicting visa requirements make it necessary.
Please refer to the U.S. Department of State resources for eligibility requirements, required documentation, and application instructions for each of these situations.
Name Changes
If you are applying for your passport in person using Form DS-11 and have legally changed your name, your passport may be issued in your current legal name, provided you present acceptable identification reflecting your updated name.
The name printed on your passport should match the name shown on the primary identification you submit with your application. If your identification has not yet been updated to reflect your legal name change, additional documentation may be required. Email IP-Passports@fsu.edu with questions.
Fees
At your appointment you will be required to pay two, separate fees. One to the US Department of State and one to the FSU Passport Acceptance Facility.
Some third-party websites falsely imply that paying online completes your passport application. In reality, these companies are typically charging you a fee to fill out the application on your behalf — they are not collecting your U.S. Department of State application fee. If you pay one of these websites, you will still be required to bring the appropriate check or money order to your appointment before we can accept your application.
In addition to the unnecessary fees, some unofficial websites may collect your personal information inappropriately or for fraudulent purposes.
Your passport application requires two separate payments:
- U.S. Department of State: Paid by check or money order and includes the passport application fee and any optional federal services.
- Florida State University Passport Services: Paid by credit or debit card and includes the passport execution (acceptance) fee and any optional passport photo services.
The fee tables below provide a breakdown of the costs associated with applying for a U.S. passport through Florida State University Passport Services.
The total amount you will pay depends on several factors, including:
- The age of the applicant (under age 16 or age 16 and older)
- Whether you are applying for a passport book, passport card, or both
- Whether you choose routine or expedited processing
- Whether you request optional services, such as expedited return shipping from the U.S. Department of State
- Whether you choose to have your passport photo taken by Florida State University Passport Services

US Department of State Fee
The first required payment is the U.S. Department of State application fee, which must be paid by check or money order made payable to the U.S. Department of State. Cash, credit cards, and debit cards CANNOT be accepted for this payment.
Please ensure your check or money order is written for the exact amount due. If you elect to purchase 1–3 Day Delivery (see below), do not round the total to the nearest whole dollar.

The second fee you are required to pay is the $35 execution fee, this fee is paid directly to Florida State University by credit/debit card only.

Renewing a Passport
Whether you can renew your passport online, by mail, or must apply in person depends on your most recent U.S. passport and your current circumstances. Use the information below to determine which application method is appropriate.
Option 1: renewal online
Eligible U.S. citizens may renew their passport online through the U.S. Department of State's Online Passport Renewal system.
You may qualify to renew online if:
- Your most recent passport was valid for 10 years and you are 25 years of age or older.
- Your passport was issued more than 9 years ago but less than 15 years ago.
- You are not changing your name, sex marker, date of birth, or place of birth.
- You do not need expedited processing.
- You are able to upload a compliant digital passport photo (most applicants can take this on their cellphone). FSU's Passport Acceptance Facility can also provide digital passport photos.
Please note that expedited processing is not available for online renewals.

Option 2: renew by mail
If you are not eligible to renew online, you may still qualify to renew your passport by mail using Form DS-82.
Generally, you may renew by mail if your most recent passport:
- Is undamaged and can be submitted with your application.
- Was issued when you were 16 years of age or older.
- Was issued within the last 15 years.
- Was issued in your current name, or you can submit legal documentation of your name change.
If you qualify for renewal by mail, the FSU Passport Acceptance Facility cannot accept your application, as renewals are mailed directly to the U.S. Department of State by the applicant.
Our office is happy to:
- Provide the appropriate renewal form and answer any questions.
- Take your passport photo for $10.74 (appointment required).
Instructions on how to submit and pay the application fee for your mail-in renewal can be found on the DS-82 form’s instruction pages (first four pages).
If You Don’t Qualify for Renewal
If you do not qualify to renew your passport online or by mail, you must apply for a new passport in person using Form DS-11 at a Passport Acceptance Facility, such as Florida State University Passport Services.
Common reasons applicants must apply in person include:
- Your previous passport was issued when you were under age 16.
- Your passport was lost, stolen, or significantly damaged.
- Your passport was issued more than 15 years ago.
- You do not meet the eligibility requirements for online or mail renewal.
If you are unsure which application method is right for you, we recommend contacting the FSU Passport Acceptance Facility before scheduling an appointment.
DS-5504: No-Fee Passport Corrections and Updates
Form DS-5504 is used to correct certain passport issues or request a replacement passport in specific circumstances. If one of the situations below applies to you, you may be eligible to receive a replacement passport without paying a new passport application fee, provided you apply within one year of your passport's issuance.
Correcting a Passport Error
Occasionally, the U.S. Department of State may issue a passport containing a printing or biographical error. Examples include:
- Misspelled name
- Incorrect date of birth
- Incorrect place of birth
- Incorrect sex marker
- Other errors introduced during passport issuance
If your passport contains an error made by the U.S. Department of State, you may submit Form DS-5504 within one year of the passport's issuance to receive a corrected passport at no additional application fee.
Replacing a Limited-Validity Emergency Passport
If your passport was lost or stolen while traveling abroad, a U.S. embassy or consulate may have issued you a limited-validity emergency passport to allow you to return to the United States.
In most cases, you may use Form DS-5504 to exchange your emergency passport for a full-validity passport once you have returned to the United States. To qualify for this no-fee replacement, you must submit your application within one year of the emergency passport's issuance.
Name Changes Within One Year
If your legal name changes within one year of your passport being issued—for example, due to marriage, divorce, or a court-ordered name change—you may request a replacement passport reflecting your new legal name by submitting Form DS-5504 along with the required legal documentation.
There is no additional application fee for this service when the request is made within one year of your passport's issuance.
Not eligible for DS-5504? You may still be able to update your name using Form DS-82 (see "Renew by Mail" section) or Form DS-11 (see "Name Changes" section). Contact us for assistance determining which application is right for you.
Expedited Processing
Although there is no new passport application fee for eligible DS-5504 requests, applicants who need their replacement passport within approximately 2–3 weeks must still pay the $60 expedited processing fee. Additional optional shipping fees may also apply if faster return delivery is requested.
How to Submit Form DS-5504
The FSU Passport Acceptance Facility cannot accept or submit Form DS-5504 applications on behalf of applicants. Once you have completed your application and gathered the required supporting documentation, you must mail your application directly to the U.S. Department of State using the address provided in the DS-5504 instructions (first four pages).
Our office is happy to answer questions about the application requirements and help you determine which documentation is needed. However, applicants are ultimately responsible for mailing their completed DS-5504 application directly to the U.S. Department of State.
